If you would like to travel outside of Alaska with your device, please complete this form and give it to your local learning center or contact teacher.

CLC Open Campus Permission Slip
Parents grant permission for their student to leave campuse at lunch with this form.
CLC Extracurricular Activities Agreement
This is an annual form that must be agreed to if the student wants to participate in any CCSD sponsored activity.
CLC Extracurricular Eligibility
This form must be completed every week during participation in an extracurricular activity.
